超声乳化白内障摘除术治疗青光眼

摘要: 超声乳化白内障摘除联合人工晶状体植入术(phacoemulsification,Phaco)治疗原发性闭角型青光眼(primary angle closure glaucoma,PACG)、晶状体源性青光眼及原发性开角型青光眼(primary open angle glaucoma,POAG)合并白内障患者均有一定降低眼压、增加前房深度的效果。对于PACG合并白内障患者,Phaco较激光周边虹膜切开术(laser peripheral iridotomy, LPI)可获得更好的疗效。单纯Phaco与Phaco联合房角分离、Phaco联合小梁切除术以及单纯小梁切除术的适应证及效果不同,其术后疗效依患者术前房角状态、眼压、降眼压药物使用数量及视神经损害状态亦不同。(国际眼科纵览,2022, 46:104-112)

Abstract: Phacoemulsification (Phaco) in the treatment of patients with primary angle closure glaucoma (PACG), phacogenic glaucoma and primary open angle glaucoma (POAG) combined with cataract has a certain  effect of reducing  intraocular pressure and increasing the depth of the anterior chamber. For PACG patients with cataract, Phaco is more effective than laser peripheral iridotomy (LPI). The indications and intraocular pressure lowering effects of Phaco alone and Phaco+goniosynechialysis, Phaco+trabeculectomy and trabeculectomy were analyzed. The postoperative efficacy was different according to preoperative anterior chamber angle, intraocular pressure, the amount of intraocular pressure lowering medications and the damage of optic nerve in fundus.
